Summary:

Occurs intermittently, but when it does it happens for about 50% of attempts to run, unplugging and reconnecting device seems to help sometimes but not always:

error: failed to launch '/Users/doug/Documents/BlackPixel/Client/[redacted]/build/Debug-iphoneos/[redacted]/[redacted]' -- failed to send the qLaunchSuccess packet

Use Xcode to debug on device

This is new in 4.3.2
