Summary: While troubleshooting other issue (Radar #12282536) discovered that php-fpm.conf appears to be reloading previously loaded extensions, then generation warnings and errors. Enabling debug level logging and executing from cli reveals following errors: [11-Sep-2012 19:39:53.156037] WARNING: pid 767, fpm_stdio_child_said(), line 166: [pool devicemgr] child 768 said into stderr: "NOTICE: sapi_cgi_log_message(), line 663: PHP message: PHP Warning: Module 'apc' already loaded in Unknown on line 0" [11-Sep-2012 19:39:53.156046] WARNING: pid 767, fpm_stdio_child_said(), line 166: [pool devicemgr] child 768 said into stderr: "ERROR: fpm_php_apply_defines(), line 129: Unable to set php_admin_value 'extension'" Fix Action: Possibly incorrect fix, but will suppress errors, comment out lines 403 & 404 in /Applications/Server.app/Contents/ServerRoot/usr/share/devicemgr/php/php-fpm.conf
