Can't add bookmarks via mobile Safari

On iOS 6, I am unable to add a new bookmark to my device.

1. Click the share button at the bottom of the screen
2. Click the bookmark option in the center.
3. Bookmark title and URL are prefilled, but the folder is blank and cannot be changed.
