Podcasts continually re-download after deletion

Summary:
If a podcast is set to automatically download and keep episodes, deleting a podcast episode from the iTunes library from within a playlist results in the episode being re-downloaded on the next refresh. This happens regardless of whether the episode is marked as Played.

Steps to Reproduce:
1. Subscribe to a podcast, and set it (or the defaults) to keep all episodes. The latest episode should automatically download.
2. Ensure that the episode appears in a playlist.
3. (optional) Listen to the episode to the end, or right-click and select "Mark as Played".
4. In the playlist, delete the episode from iTunes with option-delete.
5. Refresh the podcast.

Expected Results:
None. The deleted episode should remain deleted.

Actual Results:
The latest episode is re-downloaded.

iTunes 11.1 (126), OS X

Notes:
This bug was introduced in the current version of iTunes. In previous versions, it behaved as expected.

Since updated to say the bug persisted in 11.1.1, 11.1.3, and now 11.2.1:

Behavior persists in 11.2.1, though option-delete no longer works, must use command-option-delete.

New unexpected behavior introduced: When a podcast episode IS successfully deleted (whether manually from the Podcasts view, or automatically 24 hours after being played), the episode remains in playlists rather than being removed from the library. There seems to be no way to remove the listing at will.
