TestFlight "operation_failed" error when adding new Internal Test build

Summary:
After uploading an IPA to iTunes Connect, I am unable to start "internal testing" for the build.

Steps to Reproduce:
1. Add a build to iTunes Connect
2. Go to TestFlight > Internal Testing
3. Select a "Version to test"
4. Select "Start Testing"
5. The message "Version Undefined - Build undefined" briefly appears
6. The error message "operation_failed" appears, and no testing build was added/distrubuted

Expected Results:
A testing build appears + internal testers get notified

Actual Results:
The error message "operation_failed" appears + no testing build is added + no notification is sent out to testers

Version:
OS X, Safari

Notes:
We encountered this problem on 2 different applications on the same account.
